Once this list is generated, our team can work with the physician to review and approve each patient’s enrollment in the program. Alternatively, if the physician already has a list of patients who they believe would benefit from remote monitoring, we can work with that list to ensure seamless enrollment into our program.
Does the practice have to house devices?
Our team will work closely with patients to determine their specific monitoring needs and provide them with the appropriate devices, such as blood pressure monitors, glucose meters, and pulse oximeters. We also provide detailed instructions and support to patients on how to properly use these devices in an accurate & secure manner.

99495 is a CPT code that allows for the reimbursement of transitional care management services for patients requiring “medical decision making of at least moderate complexity.” Communication between the patient and practitioner must begin within 2 business days of discharge, and can include “direct contact, telephone [and] electronic” methods.
