DALLAS - May 12, 2026 - PRLog -- ProactiveWellness Care, ProWellCare, announced a strategic collaboration with Phoenix Pharmacy to support remote patient monitoring (RPM), continuous glucose monitoring (CGM), and provider-led diabetes management. Through this collaboration, Phoenix Pharmacy will support CGM order processing, pharmacy coordination, and direct-to-patient fulfillment for eligible ProWellCare patients nationwide.

By leveraging Phoenix Pharmacy's infrastructure and focus on personalized care, ProWellCare aims to improve access to home-based monitoring tools used in chronic disease management. Phoenix Pharmacy's pharmacy services and prescription processing capabilities are expected to support a more efficient pathway for patients to receive prescribed CGM devices and supplies.

From a value-based care perspective, CGM access gives endocrinologists and primary care teams a stronger clinical support tool to manage diabetes between visits, not just react to problems after they occur.
